Athletics Forms

Athletes must have their FHSAA physical form (EL2), FHSAA Insurance & Liability form (EL3), and Shorecrest Athletics Handbook acknowledgment form completed and turned into the Athletic Office by July 1, 2020, in order to participate in tryouts/practices. New athletes and rising sixth graders must also turn in a copy of their birth certificate.

The health and safety of the School community depends upon all persons doing their part to ensure safety not just for themselves, but for the entire community. In acknowledgement of the current COVID situation, going forward, Shorecrest will require all families who plan to have their child on campus to first read and sign an additional document to have on file before coming onto campus. This includes Flexible Learning students who come on campus for athletics and extra curricular activities. The document is located in SchoolForms Online (SFO). Login with your Shorecrest-issued credentials to review and sign. 

Did you know?

A Sports physical is not the same as a Health physical. Even if you visited your doctor before school started, you may still need to have more forms completed before beginning practice with a Shorecrest Athletic Team.
